The play-off is played in a knockout format with home and away legs. Slovan, who finished the regular season in seventh place, will face tenth-placed Bohemians from Prague in the semi-finals. Meanwhile, eighth-placed Karviná will take on ninth-placed Hradec Králové. The first leg is always played at the lower-ranked team’s stadium – this rule also applies to the final. The winners of both semi-final matchups will advance to the final, which is also played over two legs.
Slovan will play their first play-off match on Sunday, 4 May, at 15:30 in Prague. The return leg will take place on Saturday, 10 May, at 13:30 at the U Nisy stadium. The matches of the second semi-final will be played on the same weekends but on alternating days. If Slovan progresses, the first leg of the final would be away (either in Hradec Králové or Karviná) on Sunday, 18 May, at 14:30. The deciding leg would then be played in Liberec on Sunday, 25 May, at 13:30.
